1. Service Description

This purchase includes:

Analysis of functional lab results (including but not limited to DUTCH hormone testing, GI-MAP stool testing, thyroid panels, and other specialty functional labs)

One (1) 60-minute virtual review session

Educational action plan outlining possible next steps

The fee for this service is $200.

This fee covers analysis and consultation time only.

The cost of the lab test itself is separate and paid directly to the laboratory provider. Samantha Aiello Health, LLC is not responsible for laboratory pricing, processing times, sample handling, lab errors, or result accuracy.

2. Scope of Practice

Samantha Aiello Health, LLC provides educational wellness consulting only.

This service:

Is not medical care

Does not diagnose disease

Does not treat medical conditions

Does not replace physician supervision

Does not prescribe medications

Does not adjust prescribed medications

Client understands that any lab interpretation is informational and educational in nature.

Client agrees to consult with a licensed physician before making any medical decisions, including starting, stopping, or adjusting medications, hormones, or supplements.

3. High-Complexity Lab Acknowledgment

Client acknowledges that specialty functional labs (including DUTCH, GI-MAP, hormone metabolite testing, and microbiome testing):

Are interpretive tools

May vary in accuracy

May produce findings that differ from conventional medical testing

May not be recognized by all medical providers

Client understands that:

Lab interpretation is not definitive medical diagnosis

Recommendations may require medical supervision

Biological individuality affects outcomes

4. Supplement & Detox Liability Disclaimer

Recommendations may include nutritional supplements, herbal support, dietary adjustments, or lifestyle modifications.

Client understands:

Supplements are not FDA-approved to treat or cure disease

Detox reactions, Herxheimer reactions, digestive changes, hormone shifts, fatigue, headaches, skin changes, or temporary symptom flares may occur

Individual responses vary

Client assumes full responsibility for:

Implementing or not implementing recommendations

Purchasing supplements independently

Monitoring personal reactions

Seeking medical care if adverse reactions occur

Samantha Aiello Health, LLC is not liable for supplement reactions or outcomes.

5. Scheduling Policy

Session must be scheduled after lab results are received

Session must be used within 60 days of purchase

Sessions do not roll over

If Client is more than 15 minutes late, session is forfeited

No-shows without notice forfeit the session

Maximum of 2 reschedules allowed with at least 48 hours’ notice

6. Refund & Chargeback Policy

Due to the time-intensive nature of lab analysis:

All sales are final

No refunds are provided once purchased

Client agrees not to initiate chargebacks or payment disputes

Client remains financially responsible once enrolled.

7. No Guarantees

No specific health outcome is guaranteed.

Results depend on:

Compliance

Medical history

Lifestyle factors

Genetics

Environmental influences

Concurrent medical treatment

8. Limited Liability

To the fullest extent permitted by law:

Liability is limited to the amount paid for this service ($200).

Medical decisions made by Client

Lab inaccuracies

Supplement reactions

Hormonal changes

Microbiome shifts

Detox symptoms

Indirect, incidental, or consequential damages

9. Indemnification

Client agrees to indemnify and hold harmless Samantha Aiello Health, LLC from any claims arising from:

Use of recommendations

Supplement implementation

Interpretation of lab findings

Decisions made following consultation

10. Confidentiality

All personal and lab information is kept confidential unless:

Required by law

Released with written consent

Publicly available through no fault of either party
